Activate Your UCINetID

Your UCInetID is a name-based user name at UC Irvine listed on your applicant portal. It consists of a 3-8 letter combination of your first and last name. A UCInetID is not a Student ID which consists of all numbers.

You will need to activate your UCInetID for Financial Aid, Housing, and Student Health, then once you matriculate you will need your UCInetID to log onto other university systems. As an applicant, you can activate your UCInetID at the following link: UCInetID Activation. You will need the following information to activate your UCInetID:

  • Your UCInetID and UCI Student ID Number (listed on the STATUS tab of your applicant portal)
  • Your birth date (mm/dd/yyyy)
  • The last four digits of your social security number
  • Remember, you will need the password you created for your UCInetID to log into most university systems.

UC Irvine Student Housing

UC Irvine Student Housing is now accepting applications! Applications are processed in the order received and include a $20 processing fee for all first-time housing applications. To maximize your housing options, complete your application today. You do not have to submit a Statement of Intent to Register (SIR) before completing your application, as this was automatically transmitted to housing when you indicated your acceptance through your applicant portal. You will however, need to activate your UCInetID (not Student ID or AAMC ID) and password to access the housing application. If you have forgotten your UCInet ID password, you can change or reset your password by clicking here. Your UCInetID can be found on the STATUS tab home screen of your applicant portal.

Statement of Legal Residence

The Statement of Legal Residence is used by the campus registrar to make a determination of your legal state of residence for assessment of tuition and fees. All UC Irvine students are required to complete this form prior to matriculation. You will need an activated UCInetID to complete this form.


Obtain a new TB test / PPD or Interferon gamma release assay (IGRA)

Each year you will need to have a current PPD or IGRA to prove your TB status. Your TB test should be dated on or after July 1, 2023. If you do not waive out of the student health insurance plan (GSHIP), your costs will be covered for required immunizations and titers performed during GSHIP coverage dates.


Student Health Insurance and Immunization Requirements

All registered student pharmacists in UC Irvine School of Pharmacy & Pharmaceutical Sciences (SPPS) are required to have health insurance. Each UCI student is automatically enrolled in the Graduate UC Student Health Insurance Plan (GSHIP), unless they can show proof of comparable coverage from another source and submit a waiver.

  • Enroll in UC GSHIP: If you want to be enrolled in UC GSHIP, you are automatically enrolled.

    • Fees for UC GSHIP insurance coverage are charged to your student ZOT account each term and will be included with your registration fees.
    • UC SHIP coverage for incoming students is effective seven (7) days prior to the start of the student’s academic term. For students entering UCI in Fall quarter 2023, the effective date of GSHIP coverage is 9-18-2023.
  • Waive UC SHIP: If you have comparable insurance and do not want to be enrolled in UC SHIP, you must apply online for a UC SHIP waiver within the posted waiver periods and by the deadline date.

The services at the Student Health Clinic (SHC) are available to all students even if you waive UC SHIP. Students with insurance coverage other than SHIP typically pay lower fees for most services as compared with community rates (see Fees for Common Services).

Immunization Requirements

The UCI School of Pharmacy & Pharmaceutical Sciences requires all incoming student pharmacists to obtain or show proof of the following Immunizations and Tuberculosis (TB) Screening.

Required Vaccinations & Screening

Required Dosage and Screening Information

Measles, Mumps and Rubella (MMR)

Two (2) doses separated by at least 28 days, with first dose on or after 1st birthday; OR positive titer (laboratory evidence of immunity to disease).

Varicella (Chickenpox)

Two (2) doses separated by at least 28 days, with first dose on or after 1st birthday; OR positive titer. History of contracting the disease does not meet compliance.

Tetanus, Diphtheria and Pertussis (Tdap)

One (1) dose of adult Tdap vaccine (additional Td or Tdap if more than ten (10) years since last Tdap).

Meningococcal

 

One (1) dose of Meningococcal Conjugate (MenACYW) on or after age 16 for all students who are ages 21 years or younger. Not required if student is older than 21 years.

Hepatitis B

  • Series of three (3) Hepatitis B vaccinations for Engerix-B OR Recombivax HB OR series of two (2) Hepatitis B vaccinations for Heplisav-B. AND
  • Provide serology to show immunity Quantitative Hep B surface antibody in mIU/mL regardless of Hepatitis B vaccine brand.
Compliance with the above health requirements is required no later than the first date of instruction of the academic term when the student is entering UCI unless they will never be on campus during a term and have self-attested as a non-covered individual.

Influenza

  • The flu vaccination is mandated for all students.
  • Your flu vaccination must be received on or after July 1st in order to be compliant. Flu vaccinations received prior to July 1st (e.g. during the prior flu season) will not be in compliance with this mandate.
  •  In accordance with the UC policy, students are expected to enter their flu vaccination date on the Student Health Patient Portal. The deadline for the flu vaccination is Dec. 1, 2023 for new students. For returning students, the deadline is October 15, 2023.
  • The policy may be updated for the Academic Year 2023-2024, so we encourage all students to visit the Student Health Center webpage for regular updates.

COVID-19

UCOP COVID-19 vaccination policy:

Tuberculosis (TB) Screening & Testing

  • Complete a TB Risk Assessment Questionnaire
  • Written proof of a current negative PPD or documentation, including lab results, of Interferon gamma release assay (IGRA).
  • Your TB test should be dated on or after July 1, 2023.
  • Compliance is required no later than the first date of instruction of the academic term when the student is entering UCI unless they will never be on campus during a term and have self-attested as a non-covered individual.

Compliance with any immunization requirement is not fulfilled unless and until the student uploads their records to the Student Health Patient Portal and they are verified. For step-by-step instructions on how to upload your COVID-19 and other immunization records; enter your vaccine dates; and complete the TB Risk Screening Form on the portal, click here >>> Student Vaccination Upload Instructions.

Student pharmacists who fail to submit this documentation will be placed on an academic hold by the Student Health Center (SHC) and cannot register for courses for the following quarter. 

Immunization and TB Compliance

For new student pharmacists, all immunization records and TB screening will need to be completed and uploaded to the Student Health Patient Portal. Student pharmacists need to submit a health clearance request to review for UC and PharmD specific requirements. Your compliance to the requirements will be reflected under “Medical Clearances.”

If you are covered by UC SHIP, all charges will be billed to/paid by UC SHIP and you’ll have no out-of-pocket fees. If you waive out of UC SHIP, then your ZOT account will be billed and you will need to submit a claim for reimbursement to your insurance carrier.

Immunization Exemption and Declination

Medical Exemptions may be considered for student pharmacists with medical conditions which may be contraindications for the following required vaccines:

  • MMR
  • Varicella
  • Tetanus/Pertussis
  • Meningococcal ACWY
  • Hepatitis B

Declination Letters may be submitted for the following vaccines, without the need for a Medical Exemption request:

  • Influenza
  • COVID-19

Student pharmacists seeking an Exemption or Declination must submit a request via the Student Health Patient Portal no later than the compliance deadline dates mentioned above.

Student pharmacists need to be aware that the experiential rotation sites have their own specific institutional immunization requirements. Thus, an immunization exemption approved by UCI’s Student Health Center may not be accepted at certain experiential rotation sites.


Student Health Insurance

As a student at UC Irvine you will be required to have health insurance coverage. As an accepted student, you are automatically enrolled in the graduate student health insurance plan (GSHIP). A full description of the GSHIP insurance plan can be found at https://shc.uci.edu/insurance/uc-ship-benefits-and-information/gship

If you choose to waive out of the GSHIP plan, then you must provide proof of health insurance coverage that complies with UC’s waiver criteria. Details regarding GSHIP waiver criteria, the waiver process, and the online waiver application can be found at: https://shc.uci.edu/insurance/waiving-ship/apply-for-waiver The links to the 3rd party waiver vendor, Academic Health Plans (AHP), are activated on the first day of the relevant waiver period. A $50.00 fee will be charged to your ZOT account if you submit a waiver after the published deadline date. This fee is not charged to your account if your GSHIP waiver is ultimately denied.

Dependent Coverage

All graduate students who are enrolled in GSHIP have the option to enroll eligible dependents in a voluntary plan. The plan provides medical insurance and the option to add dental and vision benefits. NOTE: If both student parents are covered by GSHIP, their children may be covered as the dependents of either student, but not both.

Enrollment in the dependent plan is managed by Academic Health Plans (AHP) on behalf of GSHIP/UCOP and all carriers. Transcripts

Please be advised that PharmCAS does not forward transcripts to pharmacy schools.

You will need to request an official transcript with your undergraduate degree posted on it from your school to clear your provisional admission as soon as it is available.

UC Irvine requires official transcripts from the institutions from which you earned a degree. Official transcripts must include the name of the degree earned and the date the degree was conferred. Additional documents may be required for degrees earned outside of the U.S. Please see our website for details.Your official transcript(s) should be mailed to the address below:University of California, IrvinePharm.D. Electronic transcripts may also be sent directly by the Registrar of your school to Gradtranscripts@uci.edu.

NOTE: Do not send your official transcript to UCI until it is requested.

Send transcript as soon as your degree is awarded and posted on your transcript.

All records submitted to our office become the property of the University and will not be returned to the student.  If your academic records cannot be replaced, please bring them directly to our office and we will photocopy and return them to you.


Confidentiality Agreement

The Confidentiality Agreement is required of all students for access to the UC Irvine Health information system, and must be submitted by August 1. You will receive an email from DocuSign, a tool that enables legally-binding electronic signatures on documents. 

When you receive the emails from DocuSign, click on the “Review Documents” button, adopt a signature from the options presented, and then continue to carefully review and sign the documents enclosed.  This service will be used to initially collect your Confidentiality Agreement and Technical Standards (referenced below).


Disability Services

If you have a disability or if you think you may have a disability that requires assessment, there are a number of support services available to you. The Disability Services Center, which is located in the Humanities Quad, telephone 949-824-7494, assists students from time of admission to graduation.

Services include specialized equipment for loan or use, test-taking assistance, taped textbooks, large print materials, accessible parking, referral for diagnostic evaluation for learning disabilities, reader services for the blind and learning disabled, priority registration, provision for note takers and interpreters, a van for on-campus transportation, and four electrically operated vehicles for transportation to places on campus inaccessible by van. Other services include course-specific tutoring, liaison with professors for academic accommodations, special workshops such as career decision-making, and support groups for learning disabled students.

The office also operates a Disabilities and Computing Lab that provides adapted computer technology including speech output, scanning, screen magnification, Braille printout, special software for learning disabled students, and Braille and Speak machines. The Lab has adapted technology for PCs and Macs. Staff members can refer you to other services on campus that may be helpful, and they can assist you with government and community agencies, as necessary. All buildings at UCI are accessible to wheelchairs. Housing and dining facilities accommodate students in wheelchairs.

NOTE: If you are a UCI alumnus / alumna and have previously utilized Disability Services at UCI, you will still need to re-register.


Financial Aid

If you would like to request an estimate of your financial aid, please complete the following:

  1. Submit a FAFSA/Renewal FAFSA at http://www.fafsa.ed.gov/ (School Code 001314):
    • Include your and your spouse’s (if married) income and asset information.
    • Include income and asset information for your parents if you would like to be considered for scholarships/grants.

  2. IRS Data Retrieval from FAFSA or an IRS Tax Return Transcript for you and your spouse (if married)
  3. IRS Data Retrieval from FAFSA or an IRS Tax Return Transcript for your parents (if you would like to be considered for scholarship funding)

Once we have received all the required documents, we will be able to provide you with an estimate for your first year of the PharmD program.

Tuition/Fee Disclaimer

Some or all instructions for all or part of the Academic Year may be delivered remotely. Tuition and fees have been set regardless of the method of instruction and will not be refunded in the event instruction occurs remotely for any part of the Academic Year. Figures for tuition and fees represent currently approved or proposed amounts and may not be final. Actual tuition and fees are subject to change by the University of California as determined to be necessary or appropriate. Final approved tuition and fee levels may differ from the amounts presented.

Federal Loans Requirement (for Students Receiving Federal Aid)

Entering PharmD students are required to complete the following loan documents at www.studentloans.gov in order to receive their federal financial aid:

  • Online entrance counseling
  • Master Promissory Note (MPN) for the Direct Unsubsidized and Graduate PLUS loans

Students must complete these documents by June 30 to ensure their aid is processed before the start of school.

The purpose of the Federal Loans Entrance Counseling is to share with you everything you are required to know about the loans you are borrowing to finance your medical school education. The entrance counseling also covers a few other strategies relevant to financing your education and to successfully manage your money as a student. It is strongly recommended that you complete the entrance counseling at the time that you receive your financial aid package, as it will help you to understand the types of loans that are included in your financial aid package.

Please allow 30-45 minutes to complete an online entrance counseling session and the MPNs.

Student Complaint Policy


Complaints regarding PharmD curriculum, faculty, student affairs, or other issues over which the SPPS has jurisdiction and which are related to ACPE accreditation standards, should be filed in writing to the SPPS Office of Pharmacy Student Affairs. The Associate Dean for Pharmacy Student Affairs will review the case and meet with the student to investigate the allegation(s). If deemed necessary, the Associate Dean may refer the case to the appropriate campus entity, such as the SPPS Dean, the Office of Equal Opportunity and Diversity, etc. If informal discussions do not resolve the complaint, the case will be forwarded to the PharmD Academic and Professional Oversight Committee for review and consideration. The committee’s recommendations will be forwarded to the SPPS Dean, and the student provided with a written response. All written complaints will be maintained in a confidential file in the Office of Pharmacy Student Affairs. Students can also submit an anonymous complaint and place it in the PharmD Feedback Box at one of two locations: UCI School of Pharmacy & Pharmaceutical Sciences Administrative offices in Theory and one is in the Student Affairs Office in Steinhaus Hall.

Student pharmacists have the right to directly file a complaint in writing to the Executive Director of the Accreditation Council for Pharmacy Education (ACPE) for concerns over tuition and fee policies and if they feel there has been a violation of the ACPE accreditation standards or policies. Reporting Discrimination or Harassment

University of California employees are mandatory reporters, so they are required to report any complaints regarding harassment, sexual violence, child abuse, elder abuse, etc. Student pharmacists may seek alternative means for reporting these offenses, which may be more comfortable. Complaints regarding discrimination, sexual harassment, and sexual offenses may also be reported to the UCI Office of Equal Opportunity and Diversity or the UCI Police Department. For confidential support with these types of complaints, students may contact UCI Campus Assault Resources and Education (UCI CARE) or the UCI Counseling Center


UCI Safety Escort Program

As a safe alternative to walking alone at night, the CSO Safety Escort program is a 24-hour free service for members of the UCI community. Safety escorts are provided to the following locations:
